    If your student has been prescribed an Epipen/inhaler/seizure medication/insulin, please bring it to the nurse’s office on the first day of school.

    A signed medication authorization form (linked here) needs to be completed and signed by you and your student’s medical provider.

    Food Allergies: If your student will be eating lunch at school and has life-threatening food allergies, please complete and sign a diet prescription form (attached to this email) so that our nutrition services department can adequately provide safe alternatives for your child. Enjoy the rest of the summer!
